CAMBA is seeking a highly motivated and experienced Case Manager to join our team in Brooklyn, NY. As a Case Manager, you will be responsible for providing comprehensive support services to families and individuals in need, with a focus on housing and employment support.
Key Responsibilities- Client Intake and Assessment: Conduct initial intake and assessment of clients and their families to identify their needs and develop a plan to address them.
- Case Management: Work with clients to break through barriers to their goals and assist them in advocating for themselves and moving towards self-sufficiency.
- Service Coordination: Coordinate and facilitate social/peer support events, including group facilitation for clients.
- Advocacy: Act as client liaison/client advocate with outside organizations regarding matters such as education, healthcare, housing, and legal issues.
- Reporting and Documentation: Provide all required information for weekly/monthly/quarterly/annual reports and maintain accurate and up-to-date client files.
- Education: Bachelor's degree (e.g., B.A., B.S.W.)
- Experience: Minimum 2 years of experience in case management or a related field
- Skills: Excellent communication and interpersonal skills, ability to work with diverse populations, and strong organizational and time management skills
- Competitive Salary: $45,000 annually
