The Stability Housing Program offers 60 units of supportive housing for chronically homeless single adults and adult families with members experiencing serious mental health or substance use disorders, including those with co-occurring conditions.

Our dedicated staff provide case management and additional supportive services to empower clients in developing the skills and financial independence necessary for sustainable living.

Clients also receive partial financial assistance for utility expenses.

Role Overview:
Case Manager

Reporting Structure:
Reports to Program Manager

Key Responsibilities:
The Case Manager will oversee all contract-funded activities.

Responsibilities include conducting and finalizing Intakes, Assessments, Service Plans, Reassessments, Service Plan Updates, Accompaniment, Referrals, Advocacy, Housing Inspections, and Case Conferences.

Additionally, the Case Manager will ensure the completion of all program documentation and accurately enter client services into the Client Track system.

They will facilitate connections for clients to maintain primary medical care and adhere to prescribed medical and medication regimens.

Connect clients to essential services, including medical, mental health, substance use treatment, legal assistance, and entitlements.


Monitor clients' progress towards their goals through regular telephone check-ins and face-to-face visits, documenting updates through progress notes.

Act as a liaison and advocate for clients with external organizations concerning education, healthcare, housing, and legal matters.

Accompany clients to various appointments (educational, medical, social services, etc.)

Assist clients in completing applications for benefits and entitlements, while monitoring the outcomes of these processes.

Gather all necessary documentation and maintain organized client files.

Conduct at least one home visit per month.

Report any maintenance issues to the Housing Specialist.

Monitor clients' living conditions to ensure safety and stability.

Meet required monthly performance targets.

Provide all necessary information for the completion of monthly program reports.

Ensure that clients' records are always prepared for audits.

Qualifications:
Bachelor's or Associate degree with one year of relevant experience, or


Two years of relevant experience with sufficient education and technical skills to understand written and oral instructions; basic math skills for maintenance tasks; and to document completed tasks.


Additional Requirements:


Relevant experience, preferably in a mid-sized organization working with individuals with histories of homelessness, mental health challenges, and/or substance use issues.

Strong written and verbal communication skills.

Proficiency in Microsoft Office Suite.

Compensation:
$50,000 annually


Salary ranges represent the low and high end for the position & program. The salary offered will be based on various factors unique to each program and candidate, including experience, education, budget and/or program size, internal equity, skills, and other relevant factors.


Employment Status:
Full-time (35 hours per week)
