Job Summary

CAMBA is seeking a skilled Case Manager to join our team in providing supportive housing services to seniors 55+ with special needs. As a Case Manager, you will work closely with tenants to assess their needs, develop independent living plans, and connect them with community resources.

Key Responsibilities
  • Conduct initial intake and assessment of tenants' needs and develop individualized service plans.
  • Collaborate with tenants to identify and locate community resources, and make referrals to appropriate services.
  • Monitor tenants' progress toward their goals and document progress notes.
  • Participate in case conferences and provide updates on tenant progress.
  • Recommend and implement strategies to persuade tenants to participate in the service planning process.
  • Provide information and support to tenants to help them advocate for themselves and achieve self-sufficiency.
  • May act as tenant liaison/advocate with outside organizations regarding matters such as education, healthcare, housing, and legal issues.
  • May schedule appointments for tenants with referral organizations and escort them to appointments as needed.
  • May assist tenants in completing applications for benefits and entitlements, and process applications on their behalf.
  • May follow-up with tenants after successful completion of their primary goals to ensure stability.
  • May evaluate living conditions of tenants through home visits.
Requirements
  • Bachelor's degree (e.g., B.A., B.S.W.) and/or equivalent experience.
  • Must be fingerprinted and obtain Criminal History Checks (CHC) with Justice Center for the Protection of Vulnerable People with Special Needs.
  • Ability to maintain fingerprint clearance throughout the duration of employment.
  • As a Mandated Reporter, must immediately report suspected incidents/cases of abuse and neglect to the Vulnerable Persons Central Register (VPCR) Hotline and complete a CAMBA Incident Report.
