Housing Stability Case Manager

PRIMARY RESPONSIBILITIES
The Homebase Program represents a leading initiative in homelessness prevention within New York City.

This community-focused program aids households facing the risk of entering shelters by addressing their housing challenges and fostering pathways to self-sufficiency.

The Case Manager for the Homebase Program will be tasked with delivering housing support and comprehensive case management services. The individual will offer continuous social services to participants, aiming to achieve lasting housing stability.

Key Responsibilities:
1. Coordinate services and activities to equip program participants with essential resources for maintaining their housing and averting homelessness.

2. Collaborate with clients, their family members, supervisors, and additional staff to implement the service plan effectively.

3. Set goals and actions that tackle both the immediate housing crisis and its underlying factors.

4. Monitor and assess the progress of program participants in executing their service plans.

5. Discuss client cases with the Supervisor and Program Director to determine the need for financial assistance.

6. Prepare all required documentation for in-house subsidy applications on behalf of clients.

7. Provide referrals to clients for accessing community resources, public benefits, and entitlements.

8. Maintain precise record-keeping through case files and the designated computer system.

9. Engage in the arrangement, coordination, monitoring, and delivery of services to Homebase program participants.

10. Safeguard the rights of program participants and perform additional duties as assigned by Supervisors or the Program Director.

QUALIFICATIONS
- Bilingual in English and Spanish is required.

- Experience in non-profit organizations with a commitment to social equity.

- Strong communication skills with the ability to advocate effectively.

- Capacity to work independently while also collaborating within a team.

- Entrepreneurial mindset with problem-solving abilities and relationship-building skills.

EDUCATIONAL BACKGROUND
- An Associate's degree is required; a Bachelor's degree is preferred.
