Nurse Care Specialist

About This Opportunity:
Bridgewell is seeking a compassionate and skilled Licensed Practical Nurse to provide healthcare services to individuals with disabilities in a residential program. As a LPN, you will be responsible for delivering direct care, advocacy, and liaison with community healthcare providers, monitoring medical records, and medication administration.
Your Key Responsibilities:
- Provide specialized nursing care and support to individuals
- Document health care services and maintain accurate medical records
- Liaise with community agencies and treatment providers regarding medical needs and care
- Administer medications following MAP policies and procedures
Requirements:
- Graduation from an accredited LPN program
- Current Massachusetts Nursing license
- Two years of general nursing experience and/or service to individuals with developmental disabilities
- Excellent communication, problem-solving, and organizational skills
