Nursing Care Coordinator

We are seeking a dedicated Nursing Care Coordinator to join our team in our residential program. This is a unique opportunity to work closely with individuals with disabilities and their families, providing expert nursing care and support.
Job SummaryThe Nursing Care Coordinator will be responsible for:
- Providing high-quality nursing care and direct support to individuals with disabilities
- Collaborating with our residential manager and registered nurse to develop and implement individualized care plans
- Maintaining accurate medical records and documentation
- Communicating effectively with individuals, families, and other healthcare professionals
This role requires strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
RequirementsTo be successful in this role, you will need:
- A valid Massachusetts Nursing license
- Graduation from an accredited LPN program
- Two years of general nursing experience or service to individuals with developmental disabilities
