**Job Summary**

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Respite Home Care Specialist. As an LPN, you will be responsible for overseeing the health, safety, and well-being of program participants in our respite home setting. Your primary duties will include providing direct nursing care, managing medical information, and ensuring seamless communication with families, clinicians, and other stakeholders.

The ideal candidate will possess strong interpersonal, written, and verbal communication skills, as well as experience working with elderly or persons with developmental disabilities. A valid driver's license and reliable transportation are also required.

**Responsibilities**
  • Monitor the health and well-being of program participants, providing professional technical assistance as needed.
  • Manage and document medical information, referrals, reports, medication administration, and data collection.
  • Complete medical record audits and review medical tracking systems.
  • Orient and train staff on understanding participants' medical needs and following prescribed programs and plans.
  • Provide leadership and support during medical crises.
  • Ensure professional communication with families, DSS, clinicians, and others for assigned participants.
  • Communicate with medical providers regarding major issues.
  • Process human rights issues, incident, and accident reports in a timely manner according to agency policy/procedure.
**Requirements**
  • Licensed Practical Nurse registered with the MA Board of Registration in Nursing.
  • At least 1 year of experience working with the elderly or persons with developmental disabilities.
  • Strong interpersonal, written, and verbal communication skills.
  • Valid driver's license and reliable transportation.
