Residential Care Nurse Leader

We are seeking a compassionate and detail-oriented Licensed Practical Nurse to lead our care team in Group Homes.
Job Summary:
As a Residential Care Nurse Leader, you will be responsible for monitoring the health, safety, and well-being of program participants, providing direct nursing care, and overseeing medical needs. Your role will involve managing and documenting medical information, referrals, reports, medication administration, and data collection.
Schedule:
The schedule includes Wednesday staff meetings (every other week, 12:30-2:30 PM), Thursday (2:30 PM - 11:30 PM), Friday (2:30 PM - 11:00 PM), and Saturday (8:00 AM - 9:30 PM) with a total of 31/33 hours per week.
Essential Job Functions:
- Monitor participants' health and provide professional assistance as needed.
- Manage and document medical records, referrals, reports, medication administration, and data collection.
- Complete medical record audits and review medical tracking systems.
- Orient and train staff on understanding participants' medical needs and prescribed programs.
- Lead the team during medical crises and maintain communication with families, DSS, clinicians, and others.
- Process human rights issues, incident reports, and accident reports promptly.
Requirements:
- Licensed Practical Nurse registered with the MA Board of Registration in Nursing required.
- At least 1 year of experience working with the elderly or persons with developmental disabilities.
- Strong interpersonal, written, and verbal communication skills.
- Valid driver's license and reliable transportation.
Estimated Salary: $45-$60 per hour based on experience and location.
