About the Role

Bridgewell is seeking a skilled Licensed Practical Nurse (LPN) to join our residential program team. As a key member of our healthcare team, you will be responsible for providing direct care and health services to individuals living in our residential programs.

Key Responsibilities:

  1. Deliver direct specialized nursing and other day-to-day care to individuals.
  2. Maintain accurate documentation of health care services, adhering to relevant state regulations and nursing standards.
  3. Collaborate with the registered nurse and Director of Health Care to ensure exceptional patient care outcomes.
  4. Coordinate with community health care providers to meet individual medical needs and ensure seamless care transitions.
  5. Develop and deliver in-service educational sessions on various health topics.

Essential Qualifications:

  • Accredited LPN program completion.
  • Active Massachusetts Nursing license.
  • Minimum two years of nursing experience, preferably in a developmental disability setting.
  • Strong communication and interpersonal skills, with ability to build rapport with diverse populations.
