Mindoula Health Inc is seeking a skilled Population Health Outreach Coordinator to join our team. As a key member of our Population Health Program, you will play a vital role in connecting with our members, providing education, and supporting them throughout their journey to better health.

Responsibilities:
  • Conduct regular outreach and engagement with members virtually, fostering strong relationships and trust.
  • Provide education on Mindoula's programs, addressing questions and concerns in a clear and concise manner.
  • Assess members' behavioral health needs and evaluate self-harm risk, ensuring timely interventions.
  • Facilitate warm hand-offs to Case Managers for seamless transitions, ensuring continuity of care.
  • Support members and team through resource development and outreach, promoting a culture of inclusivity and respect.
  • Maintain accurate documentation for optimal member treatment, adhering to regulatory requirements.
  • Complete case notes and data entry promptly, ensuring timely reporting and analysis.
  • Represent Mindoula Health Inc professionally and ethically, upholding our values and mission.
  • Offer suggestions for program improvement with respect and compassion, driving positive change.
  • Collaborate with the team to achieve goals, promoting a culture of teamwork and collaboration.
  • Communicate clearly with members via phone and email, ensuring effective communication and resolution of issues.
  • Respond positively to supervision for personal growth, embracing opportunities for development and improvement.
  • Manage high-stress situations while maintaining healthy responses, prioritizing self-care and well-being.
  • Interact respectfully with members, fostering inclusivity and promoting a culture of respect.
  • Assist with outreach efforts in Population Health and other programs, expanding our reach and impact.
  • Perform additional assigned duties, embracing growth opportunities and challenges.
Requirements:
  • Previous experience in sales, outreach, customer service, engagement, and/or enrollment campaigns.
  • Proficiency in data entry and database management, with a strong understanding of data analysis and reporting.
  • Minimum of 6+ months experience in a high-volume call center, with a proven track record of success.
  • A High School Diploma or equivalent years of experience, with a strong foundation in communication and interpersonal skills.
  • Proficiency in Microsoft Suite and Google Suite (calendar, docs, sheets, slides, etc.) and with Apple Products, with a strong understanding of technology and its applications.
  • Strong written communication skills with the ability to think critically and problem-solve independently, with a strong focus on customer service and member satisfaction.
Preferred Qualifications:
  • An associate's degree from an accredited college or university, with a strong foundation in healthcare and behavioral health.
  • Experience in the behavioral health or healthcare field, with a strong understanding of population health and outreach strategies.
  • Experience working with high-risk, complex adults, with a strong focus on crisis intervention and management.
  • Completion of Crisis Intervention Training/Crisis Training, with a strong understanding of crisis management and response.
  • Familiarity with Electronic Medical Records systems, with a strong understanding of data management and analysis.

Work shifts scheduled between 11am - 8pm (MST, PST, EST, CST) not to exceed 40 hours weekly, with up to 20% mornings and weekends.
