Job Summary

We are seeking a highly organized and detail-oriented Medical Records Coordinator to join our team at Unity Health Care. The successful candidate will be responsible for ensuring the accurate and timely processing of patient medical records and referrals.

Key Responsibilities
  • Screen, track, and obtain patient referrals and medical records to ensure consult reports are received and properly reviewed.
  • Act as a liaison between hospitals and specialists to obtain reports and maintain effective communication.
  • Utilize team-based communication strategies to close the loop on referrals and ensure seamless patient care.
  • Reschedule missed appointments and maintain accurate internal and external fax inbox phone numbers.
  • Assist with population management outreach to patients by phone and maintain accurate patient information.
  • Perform other duties as assigned to ensure the smooth operation of the Health Information Management department.
Requirements
  • High school diploma or equivalent.
  • One year of clinical office experience.
Preferred Qualifications
  • Ability to understand medical terminology.
  • Ability to work independently and as a team member.
  • Good organization and communication skills.
  • Demonstrated ability to effectively work and communicate with diverse populations.
