Job Summary

We are seeking a highly motivated and experienced Facility Outreach Manager to join our team at Modivcare. In this role, you will be responsible for acting as an advocate within the non-emergency medical transportation system for patients and case managers.

Key Responsibilities
  • Provide in-service training to facilities on how to use NEMT services and promote relationships.
  • Partner with nursing homes, dialysis clinics, and other medical providers to maximize scheduling coordination and information sharing.
  • Advocate within the non-emergency medical transportation system for patients and case managers.
  • Develop and implement outreach strategies to ensure facility outreach and training are conducted throughout assigned areas of responsibility.
  • Analyze subscription service requests (standing orders) and provide education and outreach to avoid or resolve problems and increase quality.
  • Collect and analyze feedback from the community and stakeholders.
  • Provide training and development of the TripCare portal.
  • Support and guide more junior team members.
  • Participate in other projects or duties as assigned.
Requirements
  • Bachelor's Degree required.
  • Five (5) plus years of member service, transportation service, UR Manager in a health plan or social worker experience.
  • Medical transportation management experience preferred.
  • Facility-based social service programs, including facility discharge planning experience preferred.
  • Clinical, case management, and discharge experience preferred.
  • Or equivalent combination of education and/or experience.
