Job Summary:

Crossroads Health is seeking a passionate and empathetic individual to join our team as a Community Based Case Manager. As a leader in the behavioral health field, we provide a continuum of quality life-changing services to children, adolescents, young adults, adults, and families.

Key Responsibilities:

  • Provide therapeutic interventions in the home or community and link clients to resources.
  • Assess clients' development, psychological, and psychiatric needs to make referrals.
  • Advocate on behalf of clients.
  • Monitor clinical symptoms.
  • Function as a gatekeeper for other resources.
  • Perform crisis intervention as needed.
  • Meet monthly productivity metrics.

Requirements:

  • High School Diploma or equivalent, Associate's Degree preferred, with 3 years of related experience.
  • QMHS/QBHS within 15 days of hire (employer provided).
  • Demonstrated passion to advocate on behalf of others.
  • Demonstrated self-starter and independent thinker who can work in the field as well as in the office.
  • Organized with good time management skills necessary.
  • Must have an acceptable driving record (4 points or less), reliable transportation, and carry automobile insurance with minimum liability of $100,000/$300,000, and property damage of $50,000.
  • Work in home and community, frequent travel within Lake County, and providers travel to surrounding counties as needed.
  • Must be comfortable driving on highways and making home visits throughout the community.
