Position Overview:
We are seeking a dedicated professional to support aging individuals and those with disabilities. This role encompasses case management, coordinated care, and caregiver assistance in collaboration with various state agencies.

Key Responsibilities:
As a Case Manager, you will engage with the Office of Healthy Aging to provide essential case management services. Your duties will include:

  • Performing comprehensive in-home evaluations to assess care requirements.
  • Developing personalized care plans tailored to individual needs.
  • Working alongside clients, families, and service providers to ensure optimal care delivery.
  • Advocating for necessary services to fulfill client needs.

Qualifications:
To be successful in this role, candidates should possess:

  • An Associate's degree in social work or a related discipline; a bachelor's degree is preferred.
  • A minimum of one year of experience in case management.
  • Experience working with the elderly population is advantageous.
  • Bilingual proficiency in Spanish is a plus.
  • Excellent organizational and time-management abilities.
  • Proficiency in basic computer skills and familiarity with Microsoft Office 365.
  • Strong communication skills to engage effectively with a diverse clientele.
  • A collaborative spirit, along with a valid driver's license and dependable transportation.
  • A willingness to undergo background checks and medical clearances.

Physical and Travel Requirements:
This role involves various physical activities such as sitting, standing, walking, and driving, as well as lifting items up to 25lbs. Proficiency in using computers and phones is also required.
Travel: Statewide travel is necessary for this position.
