WHAT YOU WILL BE DOING (KEY RESPONSIBILITIES)...
We are in search of a compassionate and dedicated Street Outreach Case Manager. This role serves as the primary contact for clients, focusing on strategies to engage individuals on the streets and facilitating their transition to housing or shelter with the assistance of community partners.
Key Responsibilities:
- Conduct street-based outreach, navigation, and supportive services for individuals experiencing homelessness.
- Assist clients in accessing community resources while fostering professional and positive relationships with staff, clients, and community providers.
- Implement housing-first and harm reduction advocacy models with clients, ensuring respectful and courteous interactions.
- Actively listen to client needs and concerns, addressing issues fairly and professionally while coordinating with supervisors.
- Build and maintain strong connections with the community, law enforcement, and other service providers for the homeless.
- Guide clients to suitable bridge/interim housing programs and support them in completing necessary paperwork for permanent housing-related programs.
WHAT WE EXPECT FROM YOU...
- A minimum of 2 years of experience providing street-based services to vulnerable populations.
- Experience in street outreach is preferred.
- Outstanding communication skills, both verbal and written, with the ability to engage a diverse range of individuals.
- Empathetic and caring disposition, with a genuine desire to assist those in need.
- Self-driven and independent, with the ability to prioritize tasks effectively.
- A valid Driver's License and reliable transportation for travel to various facilities.
