Position Overview:


AltaPointe Health's Adult Residential Services cater to individuals facing serious and persistent mental health challenges, requiring comprehensive social rehabilitation and community support.

Our dedicated team integrates medical, nursing, and clinical services to provide essential care and treatment for over 200 adults in need of intensive support.

The Adult Residential Services Program includes group homes that are supervised around the clock when residents are present.

The primary goals for each adult group home are:
to avert re-hospitalization and to enhance and sustain successful community integration. Our residential care emphasizes rehabilitation and the reintegration of individuals with severe mental health conditions into the community.

Key Responsibilities:


Nurses in the residential environment are tasked with administering medications, reviewing Medication Administration Records (MARs) to ensure effective medication management, de-escalating or resolving crisis situations within the group home, and maintaining compliance with the Nurse Delegation Program.

Essential Duties:

1. Administers medications while adhering to the Seven Rights of medication administration, monitoring residents for potential side effects, and reporting findings to the physician.

2. Provides medication education to residents and their families, covering uses and potential side effects.

3. Reviews MARs daily to ensure appropriate medication management, including timely refills and coordination with medical staff for new prescriptions.


4. Identifies psychiatric and medical needs, effectively communicating with physicians, including completing consults and following orders.

5. Observes resident behavior, interacts with them, and evaluates needs to ensure accurate information transfer between shifts.

6. Collaborates with the multi-disciplinary team, actively participating in the development of the interdisciplinary treatment plan.

7. Monitors laboratory results, reporting any abnormal findings to the physician.

8. Demonstrates the ability to recognize crisis situations and knows how to de-escalate or resolve them, including providing emergency care as necessary (CPR, MindSet, PRN Medications).

9. Conducts infection control training for residents.

10. Ensures ongoing compliance with the Nurse Delegation Program, including recertification requirements and taking MAS calls.

11. Assists residents with personal care needs (bathing, toileting, and other hygiene issues).


12. Ensures that AIMS assessments are completed every 90 days, psychotropic consents are maintained, and annual TB skin tests are current.

13. Seeks clinical supervision and consultation as needed.

14. Accepts and implements suggestions for improvement.

15. Actively works to enhance clinical skills.

16. Attends regularly scheduled staff meetings.

17. Documents nursing services provided to residents and general observations in a timely and clear manner per AltaPointe policy.

18. Treats residents with dignity, respect, and compassion, maintaining their privacy and confidentiality.

19. Collaborates pleasantly and cooperatively with colleagues.

20. Assists residents and visitors as needed, ensuring personal values do not hinder the ability to relate and care for others.

21. Remains sensitive to the needs, expectations, and individual differences of residents.

22. Engages in Performance Improvement activities and participates in AltaPointe committees as required.

23. Completes assigned tasks promptly and accesses appropriate community resources based on resident needs.

24. Complies with AltaPointe Policies and Procedures in a timely manner.

25. Maintains current state license and fulfills renewal requirements (i.e., CEUs), attending necessary in-services and workshops.

Physical Requirements:

Frequent standing is required for retrieving files, paperwork, and distributing medications.


Sitting is common, typically lasting 5-10 minutes, but can extend up to 1.5 to 3 hours for documentation.

Walking various distances on the premises may occur frequently, up to 300 feet to dispense medications.

Frequent squatting and/or lunging are necessary for stocking supplies and retrieving materials from file cabinets and shelves.


A strong grasp is needed to manage large carts while accessing consumer files, with lighter grasp required for stocking supplies, necessitating a force exertion of up to 10 pounds.

Fine manipulation for writing, handling paperwork, and dispensing medications is essential.


Pushing and/or pulling with a force of 1-5 pounds may be required to open file cabinets and place binders on shelves.

Lifting various weights from the floor to overhead, typically ranging from 1-5 pounds, is frequent.

Occasionally to frequently, lifting 40-50 pounds from floor to waist is necessary.

Additional duties may be assigned by the supervisor.
