AltaPointe Health is seeking a highly motivated and compassionate Residential Care Coordinator to join our team. As a key member of our Adult Residential Services team, you will play a vital role in providing support and care to individuals with serious and persistent mental illness.

Key Responsibilities
  • Assist in organizing and implementing program activities for residential group homes, promoting a safe and supportive environment for our consumers.
  • Oversee money management, ensuring that consumers' financial needs are met and their independence is maintained.
  • Coordinate transportation arrangements for consumers, ensuring timely and safe transportation to appointments and other activities.
  • Assist in completing necessary reports, including vehicle and facility inspections, to ensure compliance with regulatory requirements.
  • Provide coverage and direct services as needed, including serving as a program supervisor when required.
  • Review medication management records to ensure accurate and timely refills, and collaborate with medical staff to address any medication-related concerns.
  • Assist in applying for consumers' benefits, ensuring that they receive the support they need to maintain their independence.
  • Demonstrate a proactive approach to de-escalating or resolving crisis situations, prioritizing the safety and well-being of our consumers.
  • Effectively collaborate with treatment team members and outside sources to deliver high-quality clinical services.
  • Advocate on behalf of consumers to secure community resources, promoting their independence and self-sufficiency.
  • Assist with discharge planning, ensuring a smooth transition to a less restrictive environment.
  • Document all interactions and services provided in a timely and accurate manner, adhering to AltaPointe Health's policies and procedures.
Requirements
  • Bachelor's degree in a mental health service-related field, with one year of experience in a direct mental health service-related field area.
  • Bachelor's degree not in a mental health service-related field, with two years of experience in a direct mental health service-related field.
  • High school diploma/GED, with three years of experience working directly in a mental health residential setting.
Physical Requirements

The successful candidate will be required to perform tasks that involve frequent standing, walking, and lifting, as well as occasional sitting and bending. They will also be required to handle groceries and supplies, and may be required to climb step ladders to access higher shelves.
