Job Summary
We are seeking a skilled and compassionate Registered Nurse to join our Hospice team as a Case Manager. As a RN Case Manager, you will be responsible for delivering high-quality end-of-life care to patients and their families, while also providing support and guidance to our interdisciplinary team.
Key Responsibilities
- Conduct initial nursing evaluations to determine patient eligibility for hospice services
- Develop and implement individualized plans of care to meet the unique needs of each patient
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination
- Provide education and support to patients and families on self-care techniques and disease management
- Document patient progress and outcomes in a timely and accurate manner
Requirements
- Current RN licensure in the state of practice
- One year of clinical experience in hospice or palliative care
- Current CPR certification
- Valid driver's license and access to a dependable vehicle
