Position Overview

The Licensed Vocational Nurse (LVN) Case Coordinator at Mission Community Hospital plays a crucial role in delivering empathetic and top-notch care to patients. This full-time position involves acting as a vital link among patients, their families, and the healthcare team.


Role Summary:

The LVN Case Coordinator will report to the Director of Case Management and will be a key member of the healthcare team, ensuring that patients receive the necessary support and resources throughout their care journey.

Key Responsibilities:
  • Perform initial and ongoing evaluations of patients' medical, psychological, and social requirements.
  • Create personalized care plans in collaboration with the interdisciplinary healthcare team.
  • Track and assess patient progress, providing regular updates to the healthcare team.
  • Facilitate communication among patients, families, and healthcare professionals.
  • Advocate for patients' needs, ensuring access to appropriate resources and services.
  • Educate patients on disease management, treatment alternatives, and self-care strategies.
  • Conduct regular home visits and follow-up calls to maintain continuity of care and address any emerging concerns.
  • Ensure adherence to all relevant state, federal, and hospital regulations and policies.
  • Accurately document all patient interactions and care plans in electronic medical records.

Qualifications:
  • Current LVN licensure in California.
  • A minimum of one year of experience in a clinical environment, ideally in case management or care coordination.
  • Excellent communication, organizational, and analytical skills.
  • Familiarity with various medical conditions, treatments, and available resources.
  • Ability to work autonomously as well as collaboratively within a team.
  • Proficiency in electronic medical record systems.
  • Bilingual in English and Spanish is preferred but not mandatory.
