Position Overview

About Us: Meals on Wheels Orange County is the foremost nonprofit organization dedicated to providing nutritional and supportive services for vulnerable senior citizens in the region. Our mission is to alleviate hunger and social isolation among older adults through the delivery of healthy meals, friendly interactions, and safety assessments, while also offering support to families through daytime services. Each year, we serve nearly 2 million meals to 20,000 at-risk seniors.

Role Summary: We are in search of a committed and empathetic Program Coordinator to become a vital part of our leadership team. Reporting to the Senior Manager, the Program Coordinator will oversee program elements aligned with the organization's strategic objectives. This position encompasses program leadership, management, external relationship cultivation, and knowledge sharing. The Program Coordinator will foster connections within the community to enhance resources and information accessible to eligible clients.

Key Responsibilities:

  • Evaluate program effectiveness, ensuring referral objectives and targets are achieved.
  • Advocate for program advantages and identify potential funding opportunities.
  • Supervise case documentation, monthly performance metrics, and financial records.
  • Recruit, assign, guide, and assess the performance of departmental staff.
  • Collaborate with various organizations to ensure seamless client experiences and avoid service overlap.
  • Manage relationships with external care providers and referrals, enhancing care continuity.
  • Assess client eligibility for services and connect them with suitable support options.
  • Conduct individualized needs assessments, identify challenges, and establish immediate and long-term objectives.
  • Organize and coordinate a network of supportive services in accordance with client care plans.
  • Maintain documentation and reports in compliance with departmental, agency, local, state, and federal regulations.
  • Provide training for staff and conduct home visits.
  • Coordinate with hospital staff on discharge planning.
  • Engage in meetings to disseminate program information.
  • Perform additional duties and special projects as assigned.

Qualifications:

  • Certification as a medical assistant preferred or related educational credentials.
  • A minimum of two years of experience in case management.
  • Experience working with older adults, seniors, and individuals with disabilities.
  • Exceptional communication, writing, active listening, and interpersonal skills.
  • Comprehensive understanding of case management principles and methodologies.
  • Ability to uphold professional and confidential standards.
  • Strong analytical and problem-solving abilities.
  • Familiarity with programs and services available for seniors in the region.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
  • Solid understanding of cultural competency with the target demographic.
  • Must successfully pass a background check.
  • Possession of a valid driver's license and insurance.

Physical Demands & Work Environment:

  • This role operates in a dynamic, multi-tasking, professional setting, necessitating continuous learning and complex problem-solving.
  • Ability to sit for extended periods and utilize both hands and fingers for typing.
  • Capability to operate office equipment such as copiers, printers, filing cabinets, and telephones.
  • Occasional lifting and moving of office equipment and materials up to 10 pounds.
  • Frequent computer use requires close vision.
  • The noise level in the work environment is typically moderate.
  • Occasional overtime may be required.
  • Local travel may be necessary up to 50% of the time.
