Candidate Profile:

We seek an LPN who will assess, plan, implement, and evaluate patient care while effectively collaborating with patients, families, and interdisciplinary team members, all while upholding professional nursing standards. Qualifications for LPN:

Applicants must be a Registered LPN in the State of Ohio with no board actions and possess at least one year of nursing experience.
  • Current CPR Certification.
  • Certification in Hospice & Palliative Medicine (CHPN) is preferred and must be obtained within two years of employment (eligibility requires completion of 500 hours in hospice/palliative care within the last 12 months).
  • Proficient computer skills for accurate documentation of services and care.
  • Ability to drive in various weather conditions, including nighttime.
  • Valid Driver's License with a safe driving record.
  • State minimum automobile insurance coverage.
