Key Responsibilities- Deliver post-acute care to residents, including monitoring their physical and mental wellbeing and tracking vital signs.
- Perform intakes of residents, documenting their health concerns and medical histories.
- Review medical records and charts to ensure accurate and up-to-date information.
- Administer medications and verify their accuracy.
- Maintain a safe and comfortable environment for residents, including assisting with IV set up, vaccinations, and phlebotomies.
- Contribute to the development and implementation of care plans for residents.
- Collaborate with medical staff to provide orientation and training.
- Current Ohio state license to practice as a Licensed Practical Nurse (LPN).
- Completion of an approved Licensed Practical Nurse (LPN) educational program.
- High School diploma or equivalent.
- Ability to lift a minimum of 50 pounds.
- Minimum of 6 months of experience as an LPN or in a related role.
- Competitive hourly pay range: $30-$58.
