Responsibilities

  • Provide direct patient care, implementing current nursing practice and documenting services accurately and timely.
  • Assess and provide patient and family/caregiver education, ensuring they are informed and empowered to make decisions about their care.
  • Participate in the coordination of hospice services, identifying patient needs and reporting to the interdisciplinary group.
  • Provide pain and symptom management, evaluating patient responses to treatments and medications.

Requirements

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years of nursing experience, with a preference for community health, hospice, or medical/surgical experience.
  • Currently licensed as an LPN/LVN in the practicing state.
  • Must possess current CPR Certification.
  • Demonstrates good verbal and written communication, and organization skills.
  • Valid driver's license with an automobile that is insured in accordance with state requirements.
  • Understands the philosophy of hospice care and the needs of the terminally ill.
