Licensed Practical Nurse for Hospice Care

The LPN will be responsible for planning and delivering patient care through the nursing process, which includes assessment, planning, interventions, implementation, and evaluation. The LPN will engage effectively with patients, families, and interdisciplinary team members while upholding professional nursing standards. We are dedicated to offering superior care and services to patients at the end of life, and a heartfelt commitment to hospice care is crucial.
Qualifications for LPN Candidates:
Applicants must be a Registered LPN in the State of Ohio with no disciplinary actions. A minimum of one year of nursing experience is required.
- Current CPR certification.
- Certification in Hospice & Palliative Medicine (CHPN) is preferred and must be obtained within two years of employment (eligibility requires 500 hours of hospice/palliative experience within the last 12 months).
- Proficient computer skills for accurate documentation of services and care.
- Ability to drive in various weather conditions.
- Valid Driver's License with a clean driving record.
