Essential Responsibilities of the Personal Care Specialist:

  • Reviews the Plan of Care before each visit to ensure accurate care delivery to assigned patients.
  • Provides or assists with personal care tasks, including bathing, dressing, oral hygiene, hair care, meal preparation, feeding, transfers, and exercises as outlined in the care plan.
  • Maintains a comfortable and clean environment by adjusting lighting, room temperature, changing bed linens, and performing light housekeeping as necessary. Remains vigilant regarding safety and accident prevention.
  • Timely and accurately reports vital signs and other equipment readings to the RN as directed by the care plan.
  • Acts as a member of the interdisciplinary team, communicating any patient changes or updates to the RN to ensure prompt and quality care.
  • Documents all activities in the electronic health record at the point of care.
  • Adheres to the individualized plan of care, documenting comprehensive observations, interventions, and all communications with the supervising RN.
  • Recognizes and promptly reports any changes in the patient’s mental or physical status to the RN to facilitate effective communication among team members.
  • Assists in various activities and departments as needed to ensure comprehensive patient care.
  • Engages in Continuous Quality Improvement initiatives to enhance the quality of services provided.
  • Mentors new staff to foster professional relationships with newly hired team members as required.

Qualifications for the Personal Care Specialist:

  • High School Diploma or GED
  • Certified State Tested Nursing Assistant in the State of Ohio with no board actions
  • Preferred one year of relevant experience
  • CHPNA certification required or to be obtained after completing the necessary hours for certification
  • Proficient computer skills for accurate documentation of services and care
  • Ability to drive during various conditions, including daytime, nighttime, and inclement weather.
  • Valid Driver's License with a safe driving record
  • State Minimum Automobile Insurance Coverage
