Work Schedule: Monday to Friday, 8 AM to 5 PM, with two holidays on-call each year.

Key Responsibilities:

  • Take primary responsibility for a caseload of patients and their families, encompassing assessment, eligibility determination, admission processing, care planning, implementation, and evaluation within the nursing process.
  • Gather and evaluate essential information regarding the physical, psychological, social, and spiritual aspects that may influence the needs of patients and their families, initiating interventions by other interdisciplinary team members as necessary.
  • Facilitate communication with the attending physician, interdisciplinary team, and external agencies to ensure coordinated and optimal patient care.
  • Conduct regular visits to reassess patient needs and adjust care plans accordingly.
  • Perform clinical pronouncements of death and provide necessary support to the family.
  • Maintain accurate and timely patient records, charts, progress notes, and care plans.
  • Oversee and document the care provided by Home Health Aides in compliance with Federal, State, and Company regulations.

Required Qualifications:

  • Graduation from an accredited nursing program.
  • Current licensure as a registered nurse in the state of practice.
  • Possession of valid CPR and First Aid certifications.
  • Knowledge of quality assessment and assurance protocols.
  • A valid driver's license and proof of insurance are mandatory.
