Essential Responsibilities of the Personal Care Provider:

  • Review the care plan before each visit to ensure appropriate care is delivered to assigned patients.
  • Provide or assist with personal care tasks, including bathing, dressing, oral hygiene, grooming, meal preparation, feeding, transfers, and exercises as outlined in the care plan.
  • Maintain a comfortable and clean environment by adjusting lighting, temperature, changing linens, and performing light housekeeping as necessary. Remain vigilant about safety and accident prevention.
  • Timely and accurately report vital signs and other required readings to the registered nurse as specified in the care plan.
  • Collaborate as part of an interdisciplinary team and communicate any patient changes or updates to the registered nurse to ensure timely and quality care.
  • Document all activities in the electronic health record at the point of care.
  • Adhere to the individualized care plan, documenting comprehensive observations, interventions, and all communications with the supervising registered nurse.
  • Recognize and promptly report any changes in the patient's mental or physical condition to the registered nurse to facilitate effective communication among team members.
  • Assist in various activities and departments as needed to ensure comprehensive care for all patients.
  • Engage in continuous quality improvement initiatives to enhance the quality of services provided.
  • Support the training of new staff to foster professional relationships within the team.

Qualifications for the Personal Care Provider:

  • High School Diploma or GED equivalent.
  • Certification as a State Tested Nursing Assistant in Ohio with no disciplinary actions.
  • Preferred one year of relevant experience.
  • CHPNA certification required or completion of necessary hours for certification.
  • Proficient computer skills for accurate documentation of services and care.
  • Ability to drive safely during various conditions, including day and night.
  • Valid driver's license with a clean driving record.
  • Compliance with state minimum automobile insurance requirements.
