Job Overview

Position Summary

The Case Manager for the Veterans Transitional Housing Program (VTHP) is responsible for delivering essential support services to participants, assisting them in overcoming various challenges related to finances, personal issues, health, employment, and other functional barriers. The Case Manager collaborates with each Veteran to develop a personalized service plan aimed at fostering stable income, enhancing self-determination, and facilitating a successful transition to permanent housing.

Case Management Responsibilities:

  1. Collaborates with the VTHP Peer Specialist and VA Liaison to assess potential program participants.
  2. Completes necessary intake documentation promptly upon a Veteran's entry into the program.
  3. Creates detailed, client-centered case plans that outline both immediate and long-term objectives, utilizing best practices such as Housing First, Harm Reduction, and Trauma-Informed Care in partnership with the VA Liaison.
  4. Regularly monitors participant progress through scheduled meetings.
  5. Encourages participant involvement in their own planning and goal-setting processes.
  6. Connects participants with relevant community resources to help achieve their objectives.
  7. Assists participants in developing a budget to ensure effective money management for securing and maintaining permanent housing.
  8. Supports participants in adhering to program and facility guidelines to maintain their transitional housing.
  9. Promotes effective communication skills and teaches participants how to foster positive relationships with peers and staff to resolve conflicts.
  10. Engages with service providers involved with Veterans as necessary.
  11. Works closely with community Veteran service organizations to ensure a seamless continuum of care.
  12. Stays informed about public assistance programs, health insurance options, employment and training resources, independent living skills, transportation aid, and affordable housing opportunities.
  13. Maintains composure in high-pressure situations using de-escalation techniques.
  14. Provides transportation for Veterans to appointments as required.
  15. Available for on-call emergencies as needed, sharing responsibilities with the VTHP Peer Specialist.
  16. Performs additional duties as assigned by the Director of Supportive Services.

Recordkeeping & Reporting Duties:

  1. Ensures accurate entry and exit of all participants in the Homeless Management Information System (HMIS), including case notes and assessments.
  2. Collaborates with the VA Liaison for quarterly reviews and updates of goal plans.
  3. Documents and reports incidents involving emergency services or participant issues within a 24-hour timeframe.
  4. Notifies appropriate agencies of critical issues such as suicidal ideation or significant behavioral changes.
  5. Issues necessary warnings to participants as required.
  6. Maintains comprehensive client files, including case notes, progress updates, and emergency contact information.
  7. Collects additional data to meet funding and reporting requirements.
  8. Completes discharge paperwork for the VA within two days post-discharge.

Property Management Responsibilities:

  1. Conducts quarterly inspections of the property in collaboration with the VTHP Peer Specialist.
  2. Organizes quarterly fire drills and keeps evacuation plans updated.
  3. Documents property damages and coordinates repairs as needed.
  4. Assists in recruiting and overseeing Resident Managers to ensure their responsibilities are met.
  5. Maintains cleanliness and compliance with city codes on the property.
  6. Facilitates conflict resolution among clients as necessary.
  7. Shows available rooms to prospective program participants.
  8. Handles all check-in and check-out processes with participants, ensuring timely paperwork submission.

Personal Development:

  1. Exhibits professional behavior and maintains boundaries in all interactions.
  2. Participates in biweekly supervision sessions with the Director of Supportive Services.
  3. Establishes personal development goals and tracks progress.
  4. Engages in ongoing training and maintains records for quarterly reporting.
  5. Stays informed about current issues and practices affecting individuals experiencing homelessness.

Team Collaboration:

  1. Works collaboratively with program staff and other agency personnel to foster a team-oriented environment.
  2. Demonstrates effective communication skills in building relationships with clients and colleagues.
  3. Develops positive working relationships with other service providers.
  4. Contributes to the strategic planning and implementation efforts of Porchlight.

Qualifications:

  1. Bachelor's Degree in Social Work or a related field is preferred, or equivalent experience.
  2. Military experience is advantageous but not mandatory.
  3. Experience with individuals facing mental health and substance use challenges is preferred.
  4. Proficient in Microsoft Office Suite and adaptable to new technologies.
  5. Familiarity with the Homeless Management Information System (HMIS) is a plus.
  6. Must possess a valid driver's license and have access to an insured vehicle.

Work Schedule:

This is a full-time position requiring 40 hours per week, typically Monday through Friday.
