Job Overview

Position Summary

The Veterans Transitional Housing Program (VTHP) Case Manager is responsible for delivering essential support services to participants, aimed at overcoming various barriers such as financial, personal, health, and employment challenges. The Case Manager collaborates with each Veteran to formulate a personalized service plan that fosters stable income, enhances self-determination, and facilitates a successful transition to permanent housing.

Case Management Responsibilities:

  1. Collaborates with the VTHP Peer Specialist and VA Liaison to evaluate potential program participants.
  2. Completes necessary intake documentation promptly upon a Veteran's entry into the program.
  3. Creates detailed, client-centered case plans that outline both immediate and long-term objectives, utilizing best practices in Housing First, Harm Reduction, and Trauma-Informed Care.
  4. Regularly monitors participant progress through scheduled meetings.
  5. Encourages participant involvement in their own planning and goal-setting processes.
  6. Connects participants with relevant community resources to achieve their objectives.
  7. Assists participants in developing a budget to ensure effective money management for securing and maintaining permanent housing.
  8. Supports participants in adhering to program and facility guidelines to maintain their transitional housing.
  9. Promotes effective communication skills and guides participants in building constructive relationships with peers and staff.
  10. Coordinates with service providers to address the needs of Veterans as required.
  11. Engages with community Veteran service organizations to ensure a seamless continuum of care.
  12. Stays informed about public assistance programs, health insurance options, employment and training opportunities, and affordable housing resources.
  13. Maintains composure in challenging situations by employing de-escalation techniques.
  14. Provides transportation for Veterans to appointments when necessary.
  15. Available for on-call emergencies, sharing responsibilities with the VTHP Peer Specialist.
  16. Undertakes additional tasks as directed by the Director of Supportive Services.

Recordkeeping & Reporting Duties:

  1. Manages participant entries and exits in the Homeless Management Information System (HMIS), ensuring all case notes and assessments are accurately documented.
  2. Collaborates with the VA Liaison for quarterly reviews and updates to goal plans.
  3. Documents and reports incidents involving emergency services or participant issues within a specified timeframe.
  4. Alerts appropriate agencies regarding critical issues such as suicidal ideation or significant behavioral changes.
  5. Issues necessary warnings to participants when required.
  6. Maintains comprehensive client files, including case notes, progress updates, and emergency contact information.
  7. Collects data to meet funding and reporting requirements.
  8. Completes necessary discharge paperwork promptly following participant discharge.

Property Management Responsibilities:

  1. Conducts regular inspections of the property in collaboration with the VTHP Peer Specialist.
  2. Organizes fire drills and maintains current evacuation documentation.
  3. Records any property damages or repair needs and coordinates with maintenance as needed.
  4. Assists in the recruitment and oversight of Resident Managers to ensure responsibilities are met.
  5. Ensures the property remains clean and compliant with city regulations.
  6. Facilitates conflict resolution among clients as necessary.
  7. Shows available rooms to prospective program participants.
  8. Handles all check-in and check-out processes with participants, ensuring timely submission of paperwork.

Personal Development:

  1. Exhibits professional behavior and maintains boundaries in all interactions.
  2. Engages in biweekly supervision sessions with the Director of Supportive Services.
  3. Establishes personal development goals and tracks progress.
  4. Participates in ongoing training and development opportunities.
  5. Stays informed about current trends and practices affecting individuals experiencing homelessness.

Teamwork & Collaboration:

  1. Works collaboratively with program staff and other agency personnel to foster a team-oriented environment.
  2. Demonstrates effective communication skills to build strong relationships with clients and colleagues.
  3. Establishes productive partnerships with other service providers.
  4. Contributes to the development and execution of Porchlight's strategic initiatives.

Qualifications:

  1. Bachelor's Degree in Social Work or a related field is preferred, or equivalent experience.
  2. Military experience is advantageous but not mandatory.
  3. Experience with individuals facing mental health and substance use challenges is preferred.
  4. Proficient in Microsoft Office Suite and adaptable to new technologies.
  5. Experience with Homeless Management Information System (HMIS) is a plus.
  6. Possession of a valid driver's license and access to an insured vehicle is required.

Employment Type: Full Time, 40 hours per week, Monday - Friday, 8:30 AM - 5:00 PM

On-call rotation required.
