Job Summary

Porchlight, Inc. is seeking a highly skilled and compassionate Case Manager - Housing Specialist to join our team. As a key member of our Veterans Transitional Housing Program, you will play a critical role in providing supportive services to veterans in need.

Key Responsibilities
  • Case Management: Develop and implement individualized service plans with veterans, identifying short-term and long-term goals that promote stable income, self-determination, and successful transition into permanent housing.
  • Screening and Intake: Work with the VTHP Peer Specialist and VA Liaison to screen potential program participants and complete intake paperwork within three days of a veteran entering the program.
  • Progress Monitoring: Monitor progress towards goals in weekly or biweekly meetings with participants, empowering them to become involved in their own planning and goal setting.
  • Resource Referral: Refer participants to appropriate resources to meet their goals, including community-specific resources.
  • Financial Management: Assist participants in developing a budget that ensures sufficient money management to obtain and maintain permanent housing.
  • Property Management: Perform quarterly housekeeping inspections, conduct fire drills, and maintain up-to-date documentation on evacuation plans.
  • Teamwork and Collaboration: Work collaboratively with program staff, landlords, and community agencies to facilitate a team environment and ensure satisfactory continuum of care.
Requirements
  • Education: Bachelor's Degree in Social Work or related field desired, or equivalent knowledge and experience.
  • Experience: Experience working with individuals with mental health and substance use-related concerns preferred.
  • Skills: Proficiency in Microsoft Word, Excel, Outlook, and ability to adapt to new technology.
  • Certifications: Homeless Management Information System (HMIS) experience is preferred.
  • Physical Demands: Valid driver's license, insured personal vehicle.
