Job Description

Job Summary

Porchlight, Inc. is seeking a highly skilled and compassionate Case Manager - Housing Specialist to join our team. As a key member of our Veterans Transitional Housing Program, you will play a critical role in providing supportive services to veterans in need.

Key Responsibilities

  1. Case Management
  • Work closely with the VTHP Peer Specialist and VA Liaison to screen potential program participants and develop comprehensive, client-driven case plans.
  • Monitor progress towards goals in weekly or biweekly meetings with participants and empower them to become involved in their own planning and goal setting.
  • Refers participants to appropriate resources to meet their goals, including resources in the community specific to housing, employment, and healthcare.
Recordkeeping & Reporting
  • Enter and exit all participants into the Homeless Management Information System (HMIS) and maintain accurate and up-to-date records.
  • Document and report all incidents involving emergency services, law enforcement interactions, and participant issues within 24 hours of occurrence.
Property Management
  • Perform quarterly housekeeping inspections with the VTHP Peer Specialist and conduct quarterly fire drills.
  • Document damages to property or repair needs and follow up with Maintenance as necessary.
Teamwork & Collaboration
  • Work collaboratively with program staff and other agency staff to facilitate a team environment and demonstrate effective communication skills in building relationships with clients and employees.

Requirements

  1. Bachelor's Degree in Social Work or related field desired, or equivalent knowledge and experience.
  2. Military background/experience is preferred, but not required.
  3. Experience working with individuals with mental health and substance use-related concerns preferred.
  4. Proficiency in Microsoft Word, Excel, Outlook, and ability to adapt to new technology.
  5. Homeless Management Information System (HMIS) experience is preferred.
  6. Valid driver's license, insured personal vehicle.
