ANX offers around-the-clock hospice services for individuals facing limited life expectancies or those who choose not to pursue aggressive medical interventions. Our hospice care is delivered by a specially-trained team of healthcare experts, including a medical director, social worker, skilled nursing staff, home health aides, spiritual counselors, and hospice volunteers.

Additionally, we are certified by the CMS - Center for Medicare & Medicaid Services and are a preferred partner with the Health Plan of San Mateo and Dignity Health.


Role Overview:

The Admissions Nurse serves as a registered nurse and a vital member of the Interdisciplinary Team (IDT), primarily responsible for presenting ANX Hospice Care services to patients and families, ensuring the admission of suitable patients.

This role involves conducting consultations and admission visits to evaluate patient needs, completing admission assessments, and documenting contractual agreements for hospice services.

The Admissions Nurse must possess a comprehensive understanding of hospice programs and benefits, as well as the principles of the intake, admission, and transition to care processes.


Key Responsibilities:
  • Evaluate patients for hospice eligibility, including reviewing medical records as needed.
  • Document assessments, current medications, and initiate appropriate care plans.
  • Develop and implement a plan of care that establishes goals based on nursing diagnoses, involving both the patient and their family in the planning process.
  • Assess the clinical status of patients, plan, teach, and execute immediate interventions on the day of admission.
  • Coordinate with the attending physician and Hospice Medical Director regarding patient admissions and follow-up needs.
  • Communicate admission information, issues, and concerns during patient care meetings to ensure a seamless transition and continuity of care with other team members.
  • Perform routine and unscheduled nursing visits as required and provide backup support to other field staff when necessary.
  • Utilize interpersonal skills to educate and address any questions from patients and their families regarding hospice care.
  • Participate in weekend on-call rotations, holidays, and after-hours as needed.
  • Conduct admissions at various locations, including patients' homes, hospitals, and skilled nursing facilities.

Responsibilities also include performing comprehensive nursing evaluations, demonstrating flexibility in scheduling, accepting new admissions and emergency assignments, participating in quality assurance programs, and adhering to the highest standards of professional conduct.


Qualifications:
  • Current CPR certification for Healthcare Providers.
  • A minimum of 2 years of Registered Nurse (RN) experience in Hospice Care.
  • At least 1 year of experience as a Hospice Admissions Nurse.
  • Understanding of hospice philosophy and concepts.
  • Demonstrated nursing skills and the ability to manage family emotional stress and respect individual lifestyles.
  • Self-directed with the ability to work independently.
