We are seeking an experienced Registered Nurse to join our team at ABL Health Care, LLC as a Hospice Nurse - On Call. This role requires a strong background in hospice care and the ability to provide high-quality patient care in a home setting.
Key Responsibilities- Conduct comprehensive assessments of patients and their families to determine their home care needs.
- Develop and implement individualized care plans that address the unique needs of each patient.
- Provide hands-on care to patients, including administering medications, wound care, and other treatments as needed.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Participate in on-call duties, providing 24/7 support to patients and their families.
- Document patient progress and communicate with the healthcare team to ensure continuity of care.
- Current RN licensure and CPR certification.
- At least 1 year of experience in hospice care, with a strong background in home health care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage multiple responsibilities.
- A valid driver's license and insured automobile are required.
- Current and unrestricted RN license.
- 1 year of hospice experience.
- Current BLS/CPR certification.
- Valid Driver's License.
