Key Details:
Location: New Jersey
Compensation: $53/hour
Schedule: 8:30 AM to 4:30 PM, Monday to Friday
Core Responsibilities of the Hospice Care Coordinator (RN):
- Patient Assessment: Conduct continuous physical and emotional evaluations of hospice patients, ensuring compliance with the designated care plan.
- Team Collaboration: Collaborate with the interdisciplinary team to revise nursing diagnoses and modify care plans as necessary.
- Supervision: Guide and oversee hospice aides and LPN staff, ensuring the delivery of high-quality care.
- Effective Communication: Liaise with physicians, families, and other healthcare professionals to communicate changes in patient conditions and identify needs.
- Patient Education: Offer education to patients and families, utilizing resources and materials to promote self-management and independence.
- Medication Management: Prepare and administer medications, documenting all actions in accordance with established policies.
- Record Keeping: Complete chart documentation within 48 hours, ensuring precise records of vital signs, interventions, treatments, and patient responses.
- Regulatory Compliance: Uphold infection control protocols and maintain patient confidentiality standards.
- Education: Diploma, Associate, or Bachelor of Science in Nursing; BSN preferred.
- License: Must be licensed to practice as a Registered Nurse in the State of New Jersey.
- Experience: At least 1 year of clinical experience in adult, pediatric, or medical/surgical settings (prior home care or hospice experience is preferred).
- Skills: Strong nursing assessment, documentation, and clinical skills.
- Technical Proficiency: Familiarity with Microsoft applications (Word, Excel, Outlook) and electronic medical records.
- Certifications: CPR certification is preferred.
