**Position Title**: Registered Nurse (RN) - Hospice
**Organization**: Pathways
**Location**: South San Francisco
**Patient Coverage Area**: San Carlos - San Mateo
**Employment Type**: Full Time
**Work Hours**: 8:30 am - 5 pm, with rotating weekends
**Sign-On Incentive**: $9, Conditions Apply)
The estimated compensation of $78.30/hour is provided by Pathways Home Health & Hospice in good faith. Actual earnings may differ based on various factors.
**Role Overview**: The Hospice Nurse evaluates patient needs, formulates care strategies, and collaborates with a multidisciplinary team to guarantee high-quality care delivery. Additionally, they engage in quality assessment initiatives.
**Key Responsibilities**:
- Conducts thorough patient assessments
- Records observations, interventions, and care plans
- Coordinates care with team members and other healthcare professionals
- Delivers skilled nursing care and education to patients and their families
- Participates in quality enhancement programs
**Required Qualifications**:
- Valid California RN license
- At least one year of nursing experience
- Current CPR Certification
- Valid California driver's license and insurance
