Role Summary for Admissions Registered Nurse (RN):

The Admissions Registered Nurse plays a crucial role as a member of the Interdisciplinary Team (IDT), primarily responsible for introducing ANX Hospice Care services to patients and their families, ensuring the admission of suitable candidates. Key responsibilities include:

  • Understanding hospice programs and their benefits.
  • Knowledge of intake, admission, and care transition processes.
  • Evaluating patients for hospice eligibility through medical record assessments.
  • Documenting patient assessments and current medication regimens.
  • Developing and initiating care plans in collaboration with patients and families.
  • Assessing clinical status and implementing immediate interventions upon admission.
  • Coordinating medication and durable medical equipment needs for timely delivery.
  • Collaborating with physicians and the Hospice Medical Director regarding admissions.
  • Communicating pertinent admission information during care meetings to ensure continuity of care.
  • Conducting routine and unscheduled nursing visits as necessary.
  • Utilizing interpersonal skills to educate patients and families about hospice care.
  • Participating in on-call rotations and after-hours support as required.
  • Conducting admissions in various settings, including homes and healthcare facilities.
  • Performing comprehensive nursing evaluations and adapting to new admissions and emergency assignments.
Qualifications for Admissions Registered Nurse (RN):
  • Must possess a valid Registered Nurse (RN) license.
  • Current CPR certification for Healthcare Providers.
  • Minimum of two years of nursing experience in hospice care.
  • At least one year of experience as a Hospice Admissions Nurse.
  • Valid driver's license and insured vehicle in good working condition.
  • Understanding of hospice philosophy and experience with terminally ill patients.
  • Demonstrated nursing skills and ability to manage emotional stress in families.
  • Excellent communication and problem-solving abilities.
  • Self-motivated and capable of working independently.
