Evening Hospice Care Registered Nurse (RN)

LOCATION:
South San Francisco

PATIENT SERVICE AREA:
San Mateo - San Francisco

WORK SCHEDULE:
Per Diem or Part Time

SHIFT HOURS:

4pm-12:30am. The compensation range of $78.30/Hour is an estimate that reflects the potential pay Pathways Home Health, Hospice and Private Duty may offer for this role, based on current circumstances.

Pathways Home Health, Hospice and Private Duty may ultimately adjust compensation as permitted by law.

ROLE OVERVIEW:

The After Hours Visit RN is tasked with delivering skilled nursing visits to patients and their families as directed by Intella Triage. This role involves documenting observations and interventions related to the patient's environment and overall status, while coordinating care with the Interdisciplinary Group (IDG) or team, including the patient's physician.


KEY RESPONSIBILITIES:
  • Conduct patient admissions, assessing physical, psychosocial, environmental, safety, and developmental needs to create a tailored care plan.
  • Secure informed consent from patients and families, assisting with necessary documentation for hospice admission.
  • Evaluate patient conditions, providing and documenting appropriate skilled nursing care, including education and family support.
  • Assess the patient's environment and eligibility for services in compliance with third-party regulations.
  • Coordinate care with the patient's case manager, reporting significant changes in the patient's condition.
  • Communicate with the patient's physician as needed.
  • Demonstrate nursing skills, including vein puncture and managing high interventions at home, such as IVs.
  • Maintain adequate medical supplies and agency forms.
  • Discuss the history, philosophy, and services of Pathways Home Health and Hospice knowledgeably.
  • Provide guidance to patients and families regarding healthcare decision-making and advance directives in collaboration with social workers.
  • Assess pain and other symptoms, developing individualized management plans.
  • Educate and support patients, families, caregivers, and facility staff.
  • Document assessments, care interventions, and outcomes in the electronic record.
  • Ensure timely communication with patients, families, and facility staff regarding care.
  • Participate in quality assessment and performance improvement initiatives.
  • Adhere to safety and infection control policies.
  • Maintain and update nursing knowledge and skills.
  • Perform additional duties as assigned, consistent with skills and training.

QUALIFICATIONS:
  • Valid California RN license; experience in Hospice or Home Health preferred.
  • Graduate of an NLN accredited nursing program.
  • Self-motivated, flexible, and capable of working effectively in a multidisciplinary team.
  • Strong nursing assessment, communication, and problem-solving skills.
  • Minimum one year of professional nursing experience within the last three years or a baccalaureate degree in nursing from an accredited program.
  • Valid California driver's license and automobile insurance as per agency policy.
  • Current CPR certification from the American Heart Association BLS Health Care Provider.
