Role Summary

The Nights Licensed Vocational Nurse (LVN) plays a crucial role in delivering direct patient care under the guidance of a registered nurse. Key responsibilities include adhering to care plans, administering treatments, and collaborating with team members to achieve positive patient outcomes.

Work Schedule: 9:30 PM - 8:00 AM, Monday - Thursday

Core Responsibilities

  • Assist the RN in assessing situations, anticipating potential issues, and determining appropriate actions.
  • Evaluate patient responses to care and treatments in a timely and safe manner.
  • Support field staff with electronic medical record (EMR) entries and troubleshooting.
  • Perform nursing skills as outlined in the basic skills checklist, adhering to policies, procedures, and standards of care.
  • Under RN supervision, implement and document educational plans tailored to the assessed needs of patients and families.
  • Thoroughly document all patient care provided, including communications with other staff members, following established charting standards.
  • Access and manage data from established computer systems, either on-site or remotely.
  • Orient, instruct, and train assigned personnel as needed.
  • Assist the agency with audits and other administrative tasks as assigned.
  • Receive cross-training in Clinical, Triage, Admissions, Access Center, and Program Representative roles.

Qualifications:

  • Completion of Vocational/Specialized Training.
  • Current California LVN license is required.
  • Proficient in answering phones, maintaining patient records, and scheduling appointments.
  • Intermediate proficiency in Office applications and EMR systems.
  • A minimum of 3 years of clinical experience, with hospice experience preferred.
  • Must possess a valid driver's license and have access to an insured vehicle.
