Position Overview:
Under the guidance of the Director of Business Development, the Community Outreach Coordinator will be responsible for promoting hospice and home health services to healthcare professionals, community members, and families, ensuring that our values and quality of care are effectively communicated.

Key Responsibilities:
1. Develop and nurture professional relationships with referral sources, including hospitals, physician practices, nursing facilities, and case management teams.
2. Ensure adherence to all relevant state, federal, and local regulations.
3. Communicate the advantages of home care services to both professional contacts and potential clients.
4. Educate the community on the services offered by Apreva Hospice.
5. Engage daily with healthcare professionals, including physicians, case managers, and skilled nursing staff.
6. Conduct outreach efforts to existing and prospective referral sources within designated areas.
7. Advocate for Apreva Hospice's programs and services at community events and professional gatherings.

Qualifications:
- Bachelor's degree in Business, Healthcare, or a related field is preferred.
- Valid state driver's license and proof of auto insurance.
