In this role, you will also meet with patients and caregivers to coordinate services for their transition to home or assisted living settings. Your attention to detail will be paramount in ensuring that all necessary documentation is completed accurately and submitted in a timely manner, facilitating smooth transitions and peace of mind for all involved.
