Role Summary:

The Crisis Transition Navigator (CTN) plays a crucial role in assisting Non-Title XIX and Title XIX-Non-SMI Adults who have utilized the Maricopa County crisis system. The CTN acts as a vital link to ongoing support services by coordinating and overseeing RBHA/Community-based resources, ensuring the effective delivery of necessary services and assistance. The primary objective of the CTN is to aid individuals who are willing and capable of providing informed consent in accessing post-crisis services that address needs such as food, housing, financial stability, and the establishment of support networks.

Key Responsibilities:

  • Engage with individuals receiving crisis services in the community as referred by designated crisis providers. Complete AHCCCS applications and collaborate with DES offices for those meeting AHCCCS eligibility criteria. Assist in outlining appropriate next steps for individuals requiring support in accessing various providers or community services post-crisis, including the development of a personalized Crisis Navigation Plan (CNP). The plan will be self-directed by the recipient and coordinated with the CTN, who will follow up and support the implementation of identified needs.
  • Build and maintain a comprehensive network of Community-Based and RBHA resources. The CTN must be knowledgeable about accessing a variety of social and behavioral health community services, which may include food, clothing, housing, support services, and Outpatient Crisis Stabilization/Transition services. This may also involve scheduling necessary appointments and arranging SMI evaluations and Outpatient Stabilization/Transition services when appropriate.
  • Ensure meticulous and comprehensive documentation in progress notes and within the client's Crisis Navigation Plan (CNP).
  • Act as a short-term recovery coach, assisting participants in developing and executing self-directed recovery plans, sharing personal experiences, providing support, and teaching self-management techniques.

Additional Responsibilities:

  • Facilitate appointments with treatment providers for behavioral health assessments as needed.
  • Coordinate transportation for post-crisis/stabilization needs.
  • Assist clients in obtaining essential identification documents, such as Birth Certificates.
  • Identify transportation options for clients.
  • Refer clients for SMI evaluations when applicable.
  • Coordinate housing arrangements with community shelters and housing facilities.
  • Provide recommendations to the Program Coordinator or Manager for service delivery improvements and develop specialized functions related to department services.
  • Participate in agency fundraising activities as requested.
  • Drive a company vehicle or personal vehicle as required, maintaining a valid Arizona driver's license and appropriate liability insurance.

Qualifications:

  • High School Diploma or GED is mandatory.
  • Peer Certification is required.
  • Must self-identify as a recipient of mental health services or as someone who has supported a family member in their mental health or substance abuse recovery.
  • Identification as a Military Veteran, Service Member, or Service Family Member is preferred.
  • Strong interpersonal skills to foster professional relationships with coworkers, clients, families, and community members.
  • Two years of relevant experience is preferred.
  • Effective communication skills demonstrated through a non-judgmental approach.
  • Proficient listening skills and positive interaction, with a thorough understanding of family support roles throughout the recovery process.
  • Ability to interpret and implement complex treatment plans based on a broad knowledge of available community services.
  • Must possess a Fingerprint clearance card or be eligible to obtain one.
  • Must hold a valid AZ driver's license with no major infractions in the past three years.
  • Regular access to reliable transportation is required.
  • Bi-lingual (Spanish) proficiency is preferred.
  • Must be at least 21 years of age at the time of employment.

  • Maintain professional conduct consistent with ethical standards and agency policies.
  • Foster positive working relationships with internal and external team members, stakeholders, and community partners.
  • If driving for agency purposes, a valid Arizona driver's license and appropriate liability insurance are required, along with a clean driving record.
