Role Summary:

The Crisis Transition Navigator (CTN) plays a crucial role in assisting Non-Title XIX and Title XIX-Non-SMI Adults who have utilized the Maricopa County crisis system. The CTN acts as a vital link to ongoing support services by coordinating and overseeing RBHA/Community-based resources, ensuring the effective delivery of necessary services and assistance. The primary objective of the CTN is to empower individuals who are voluntarily seeking help to access post-crisis services aimed at enhancing their food security, housing stability, financial well-being, and support networks.

Key Responsibilities:

  • Engage with individuals utilizing crisis services in the community as referred by designated crisis providers. Complete necessary applications and collaborate with relevant agencies for those meeting eligibility criteria. Assist in outlining the most suitable next steps for individuals requiring further assistance in accessing various providers or community services post-crisis, including the development of a personalized Crisis Navigation Plan (CNP). The plan will be self-directed by the recipient and coordinated with the CTN, who will follow up and support the implementation of identified needs.
  • Build and maintain a comprehensive network of Community-Based and RBHA resources. The CTN must possess knowledge of various social and behavioral health services available, which may include food, clothing, housing, support services, and Outpatient Crisis Stabilization/Transition services. This may also involve scheduling necessary appointments and arranging evaluations when applicable.
  • Ensure meticulous and comprehensive documentation in progress notes and within the client's Crisis Navigation Plan (CNP).
  • Serve as a short-term recovery coach, aiding participants in developing and executing self-directed recovery plans, sharing personal experiences, providing support, and teaching self-management techniques.

  6. Other relevant peer specialist and system training

Additional Responsibilities:

  • Facilitate appointments with treatment providers for behavioral health assessments as needed.
  • Coordinate transportation for post-crisis/stabilization needs.
  • Assist clients in obtaining necessary identification documents.
  • Identify transportation options for clients.
  • Refer individuals for SMI evaluations when appropriate.
  • Coordinate housing arrangements with community shelters and housing facilities.
  • Provide recommendations to management for service delivery improvements.
  • Participate in agency fundraising initiatives as requested.
  • Drive company vehicles as required, maintaining a valid driver's license and appropriate insurance coverage.

Qualifications:

  • High School Diploma or GED is mandatory.
  • Peer Certification is essential.
  • Must self-identify as a recipient of mental health services or as a family member involved in the recovery process.
  • Preference for candidates identifying as Military Veterans, Service Members, or Service Family Members.
  • Strong interpersonal skills to foster professional relationships with coworkers, clients, families, and community members.
  • Two years of relevant experience is preferred.
  • Effective communication skills with a non-judgmental approach.
  • Ability to listen actively and interact positively, with a thorough understanding of family support's role in recovery.
  • Capability to interpret and implement complex treatment plans based on a broad knowledge of community services.
  • Fingerprint clearance card or eligibility to obtain one is required.
  • Valid Arizona driver's license with no major infractions in the past three years is necessary.
  • Regular access to reliable transportation is required.
  • Bi-lingual (Spanish) candidates are encouraged to apply.
  • Must be at least 21 years of age at the time of employment.

  • Foster positive working relationships with internal and external team members, stakeholders, and community partners.
  • When driving for agency purposes, a valid Arizona driver's license and appropriate insurance coverage are required, along with a clean driving record.
