About the Role

La Frontera Arizona is seeking a highly skilled and compassionate Crisis Transition Navigator to join our team. As a Crisis Transition Navigator, you will play a critical role in supporting individuals who have received crisis services and are in need of post-crisis assistance.

Key Responsibilities
  • Meet with consumers of crisis services in the community to assess their needs and develop a personalized Crisis Navigation Plan (CNP).
  • Collaborate with a network of community-based resources and RBHA providers to ensure seamless access to services such as food, housing, financial stability, and support systems.
  • Develop and maintain accurate and thorough documentation of client progress and CNPs.
  • Provide short-term recovery coaching and support to help individuals develop and implement self-directed recovery plans.
  • Stay up-to-date on complex treatment plans and community services, and effectively interpret and implement them.
Requirements
  • High School Diploma or GED.
  • Peer Certification and/or experience is preferred.
  • Substance Abuse experience is preferred.
  • Must self-identify as a recipient of mental health services or having assisted a family member in their mental health or substance abuse recovery process.
  • Effective interpersonal skills, including professional and positive relationships with coworkers, consumers, families, and community members.
  • Two years of prior experience preferred.
  • Must have a valid AZ driver's license with no major infractions in the past three years.
  • Must have regular access to reliable transportation.
