About the Role

La Frontera Arizona is seeking a highly skilled and compassionate Crisis Transition Navigator to join our team. As a Crisis Transition Navigator, you will play a critical role in supporting individuals who have received crisis services and are in need of post-crisis assistance.

Key Responsibilities
  • Meet with consumers of crisis services in the community to assess their needs and develop a personalized Crisis Navigation Plan (CNP).
  • Collaborate with a network of community-based resources and RBHA providers to ensure seamless access to services such as food, housing, financial stability, and support systems.
  • Develop and implement CNPs, monitoring recipient progress and making adjustments as needed.
  • Provide short-term recovery coaching, teaching self-management techniques and promoting self-directed recovery.
  • Maintain accurate and thorough documentation in progress notes and CNPs.
Requirements
  • High School Diploma or GED.
  • Peer Certification and/or experience preferred.
  • Substance Abuse experience preferred.
  • Must self-identify as a recipient of mental health services or having assisted a family member in their mental health or substance abuse recovery process.
  • Effective interpersonal skills, including professional and positive relationships with coworkers, consumers, families, and community members.
  • Two years of prior experience preferred.
  • Effective communication skills, including a non-judgmental approach.
  • Ability to interpret and implement complex treatment plans based on a wide knowledge base of available community services.
  • Fingerprint clearance card or eligibility to obtain one.
  • Valid AZ driver's license with no major infractions in the past three years.
  • Regular access to reliable transportation.
  • Bi-lingual (Spanish) Diff per hour available.
  • Must be at least 21 years of age at time of employment.
