Crisis Support Transition Specialist

Role Summary:
The Crisis Transition Navigator (CTN) is responsible for assisting Non-Title XIX and Title XIX-Non-SMI Adults who have utilized the Maricopa County crisis system. The CTN acts as a crucial link to ongoing support services by coordinating and overseeing RBHA/Community-based resources, ensuring the effective delivery of necessary services and assistance. The primary objective of the CTN is to aid individuals who are voluntarily seeking help in accessing post-crisis services related to essential needs such as food, housing, financial stability, and support systems.
Key Responsibilities:
- Engage with individuals receiving crisis services in the community as referred by designated crisis providers. Complete AHCCCS applications and collaborate with DES offices for those meeting AHCCCS eligibility criteria. Assist in developing a tailored Crisis Navigation Plan (CNP) that outlines the most suitable next steps for individuals post-crisis, ensuring the plan is self-directed and coordinated with the CTN, who will follow up on the implementation of identified needs.
- Build and maintain a comprehensive network of Community-Based and RBHA resources. The CTN must be knowledgeable about various social and behavioral health services, including but not limited to food, clothing, housing, support services, and Outpatient Crisis Stabilization/Transition services. This includes scheduling necessary appointments and arranging evaluations when required.
- Ensure meticulous documentation in progress notes and within the client's Crisis Navigation Plan (CNP).
- Act as a short-term recovery coach, assisting participants in creating and executing self-directed recovery plans, sharing personal experiences, providing support, and teaching self-management techniques.

- *AHCCCS eligibility/Affordable Health Care Act
- *SMI eligibility
- *Crisis System
- *Other relevant peer specialist and system training
Additional Duties May Include:
- Facilitating appointments with treatment providers for behavioral health assessments.
- Coordinating transportation post-crisis/stabilization.
- Assisting clients in obtaining necessary identification documents.
- Identifying transportation options.
- Referring individuals for SMI evaluations when applicable.
- Coordinating housing arrangements with community shelters and facilities.
- Recommending improvements to service delivery to the Program Coordinator or Manager.
- Participating in agency fundraising activities as requested.
- Driving company vehicles or personal vehicles as needed, maintaining a valid Arizona driver's license and appropriate insurance coverage.
Qualifications:
- High School Diploma or GED is required.
- Peer Certification is mandatory.
- Must self-identify as a recipient of mental health services or as someone who has supported a family member in their recovery process.
- Identification as a Military Veteran, Service Member, or Service Family Member is preferred.
- Strong interpersonal skills to foster professional relationships with coworkers, clients, families, and community members.
- Two years of relevant experience is preferred.
- Effective communication skills demonstrated through a non-judgmental approach.
- Ability to interpret and implement complex treatment plans based on a comprehensive understanding of available community services.
- Must possess a Fingerprint clearance card or be eligible to obtain one.
- Must have a valid AZ driver's license with no major infractions in the past three years.
- Regular access to reliable transportation is required.
- Bi-lingual (Spanish) proficiency is preferred.
- Must be at least 21 years of age at the time of employment.

- Demonstrated ability to remain composed in crisis situations, work independently and collaboratively, and relate positively to individuals from diverse backgrounds.
- Maintain professional conduct in line with ethical practices and agency policies.
- Foster positive working relationships with internal and external stakeholders.
- Valid Arizona driver's license and appropriate insurance are required for driving during agency business.
