Role Summary:

The Crisis Transition Navigator (CTN) is responsible for assisting Non-Title XIX and Title XIX-Non-SMI Adults who have engaged with the Maricopa County crisis system. The CTN acts as a vital link to ongoing support services by coordinating and overseeing RBHA/Community-based resources, ensuring that individuals receive the necessary assistance following a crisis. The primary objective of the CTN is to empower individuals who are willing and capable of providing informed consent to access post-crisis services that address their needs for food, housing, financial stability, and support systems.

Key Responsibilities:

  • Engage with individuals utilizing crisis services in the community as referred by designated crisis providers. Complete AHCCCS applications and collaborate with DES offices for those meeting AHCCCS eligibility criteria. Assist in outlining appropriate next steps for individuals requiring support in accessing various providers or community services post-crisis, including the development of a self-directed Crisis Navigation Plan (CNP). Follow up to ensure the successful implementation of the identified needs within the CNP and monitor recipient progress.
  • Build and maintain a comprehensive network of Community-Based and RBHA resources. The CTN must be knowledgeable about various social and behavioral health services available, which may include food, clothing, housing, support services, and Outpatient Crisis Stabilization/Transition services. This includes scheduling necessary appointments and arranging SMI evaluations and Outpatient Stabilization/Transition services when applicable.
  • Ensure meticulous documentation in progress notes and within the client's Crisis Navigation Plan (CNP).
  • Act as a short-term recovery coach, aiding participants in developing and executing self-directed recovery plans, sharing personal experiences, providing support, and teaching self-management techniques.

Additional Responsibilities:

  • Facilitate appointments with treatment providers for behavioral health assessments as necessary.
  • Coordinate transportation for post-crisis/stabilization needs.
  • Assist clients in obtaining essential identification documents.
  • Identify and recommend transportation options.
  • Refer clients for SMI evaluations when criteria are met.
  • Coordinate housing arrangements with community shelters and housing facilities.
  • Provide recommendations to the Program Coordinator or Manager for enhancing service delivery and develop specialized functions related to department services.
  • Participate in agency fundraising activities as requested.
  • Driving of company vehicles or personal vehicles is required, necessitating a valid Arizona driver's license and appropriate liability insurance.

Qualifications:

  • High School Diploma or GED is mandatory.
  • Peer Certification is required.
  • Must self-identify as a recipient of mental health services or as someone who has supported a family member in their recovery process.
  • Identification as a Military Veteran, Service Member, or Service Family Member is preferred.
  • Strong interpersonal skills for fostering professional relationships with coworkers, consumers, families, and community members.
  • Two years of relevant experience is preferred.
  • Effective communication skills demonstrated through a non-judgmental approach.
  • Ability to listen actively and interact positively, with a comprehensive understanding of family support's role in the recovery process.
  • Capability to interpret and implement complex treatment plans based on a broad knowledge of available community services.
  • Fingerprint clearance card or eligibility to obtain one is required.
  • A valid AZ driver's license with no major infractions in the past three years is necessary.
  • Regular access to reliable transportation is required.
  • Bi-lingual (Spanish) proficiency is preferred.
  • Must be at least 21 years of age at the time of employment.

  • For driving during agency time or business, a valid Arizona driver's license and appropriate liability insurance are required, along with a clean driving record.
