About the C-Change Initiative

The C-Change Program is designed to transform the lives of youth at risk or identified as victims of sexual exploitation. Our approach emphasizes stabilization, reducing mental health challenges, fostering self-efficacy, and building community connections. We offer specialized mental health screenings, advocacy, clinical case management, and psychotherapy services to youth up to 21 years old across the Bay Area.

Role Summary

The Service Coordination Specialist will work closely with C-Change directors and external organizations to coordinate services for youth affected by exploitation. This position involves supporting onboarding, orientation, and cross-program collaboration among C-Change staff. Candidates must possess strong advocacy, administrative, and facilitation skills, along with creativity, self-motivation, and adaptability. As a primary contact for referral parties, the Specialist will assist in the intake process and conduct needs assessments to enhance service engagement and stabilization.

The Specialist is responsible for overseeing the care and treatment of children and families, ensuring alignment with the Child and Family Team (CFT) Plan for sustainable improvements. Utilizing a trauma-informed, strengths-based approach, the Specialist advocates for youth, ensuring their voices and choices are central to their care, promoting empowerment and effective healing steps.

Key Responsibilities:

  • Facilitate Multidisciplinary Meetings: Regularly organize and lead meetings with providers, youth, families, and natural supports to ensure effective service coordination.
  • Partner Collaboration: Work collaboratively with families and formal system partners to facilitate a strength-based, culturally sensitive service planning process.
  • Conduct Initial Assessments: Perform initial evaluations with youth and referring parties to assess needs and connect them to necessary services.
  • Provide Immediate Support: Offer initial engagement and support to youth, including case management and psycho-education on trauma impacts and exploitation.
  • Orientation and Training: Assist in delivering orientation and training on community services, program policies, and trauma-informed practices.
  • Collaborate on Interventions: Work with Human Services Agency and other providers on screening and support services for youth impacted by exploitation.
  • Supervision and Training Participation: Attend individual and group supervision, program meetings, and training sessions consistently.
  • Work Location and Travel: Be available to work from various sites and travel as required.
  • Engage Youth in Treatment Planning: Involve youth in discussing their needs and goals for treatment and care.
  • Support Program Objectives: Engage in special projects and tasks as directed by Program Directors to further C-Change goals.
  • Other responsibilities as assigned.

Qualifications:

  • Bachelor's degree in psychology, criminal justice, or a related field preferred.
  • Minimum of 3 years of experience in targeted case management for Transition-Aged Youth.
  • Experience with high-risk youth, including those affected by exploitation or severe mental health issues.
  • Knowledge of the needs and strengths of foster youth.
  • Proven ability to engage clients positively and collaborate effectively in diverse teams.
  • Familiarity with community resources and trauma-informed care.
  • Experience with Medi-Cal billing and quality assurance.
  • Strong computer skills and understanding of Diversity, Equity, and Inclusion practices.
  • Ability to work independently and within multidisciplinary teams.
  • Clean driving record; travel is required.
