The EMPOWR program is designed to assist transition-aged youth by delivering mental health support, ensuring housing stability, and facilitating educational and employment opportunities through community-based therapy and case management.

The Case Coordinator position entails delivering direct assistance to the youth involved in the program, working closely with the EMPOWR Team to provide advocacy, rehabilitation services, and necessary referrals.

Key Responsibilities:

  • Deliver community-based, trauma-informed support to transition-aged youth
  • Provide advocacy, case management, and rehabilitation services
  • Guide youth in accessing vital community resources
  • Conduct risk assessments, crisis interventions, and safety planning
  • Assist youth in obtaining essential services and support
  • Implement services within the community setting

Qualifications:

  • Bachelor's degree in Psychology, Social Work, Criminal Justice, or a related discipline preferred
  • A minimum of 2 years of experience working with youth involved in systems
  • Proven experience in case management and rehabilitation services
  • Ability to work both independently and collaboratively within diverse teams
  • Familiarity with community resources and proficiency in documentation
  • Skilled in engaging with clients who have complex trauma histories

Position Details and Compensation:

Salary: Starting at $65,000 annually ($71,500 for bilingual Spanish speakers), inclusive of benefits and performance-based incentives.
