WestCoast Children's Clinic, a non-profit organization dedicated to community mental health, is seeking a Service Coordination Specialist to enhance the lives of vulnerable youth and families in the Bay Area.
About the Role
The Service Coordination Specialist will work closely with program directors and external partners to ensure comprehensive service delivery for youth who have experienced exploitation. This position involves supporting team members through onboarding, consultations, and collaboration across various programs. Candidates should demonstrate strong advocacy, administrative, and facilitation skills, along with creativity and adaptability.
Key Responsibilities:
- Facilitate Multidisciplinary Meetings: Organize and lead meetings with providers, youth, and families to coordinate services effectively.
- Partner Collaboration: Engage with families and system partners to create a culturally sensitive and needs-driven service planning process.
- Conduct Initial Assessments: Perform evaluations with youth and referring parties to determine needs and facilitate access to necessary services.
- Provide Immediate Support: Offer case management and psycho-education in a safe environment to assist youth in their recovery journey.
- Orientation and Training: Provide training on community services and trauma-informed practices, particularly for youth impacted by exploitation.
- Collaborate on Interventions: Work with service providers on screening and support services for youth affected by sexual exploitation.
- Engage in Supervision: Participate in individual and group supervision, program meetings, and training sessions regularly.
- Travel Requirements: Be prepared to travel within a designated radius for work-related duties.
- Involve Youth in Treatment Planning: Engage youth in discussions about their treatment goals and care options.
- Support Program Initiatives: Contribute to special projects and tasks as directed by program leadership.
- Other duties as assigned.
Qualifications:
- Bachelor's degree in psychology, social services, or a related field preferred.
- Minimum of 3 years of experience in case management and rehabilitation services for youth.
- Experience working with high-risk populations, including those with mental health challenges.
- Strong ability to engage clients and collaborate within diverse teams.
- Knowledge of community resources and trauma-informed care practices.
- Proficient in computer skills and understanding of Diversity, Equity, and Inclusion (DEI) principles.
- Valid California driver's license; travel is required.
