Position Overview

The Service Coordination Specialist plays a pivotal role in the C-Change Program at WestCoast Children's Clinic, a non-profit organization dedicated to enhancing the lives of children and families in the Bay Area. This position is essential for fostering a supportive environment that is client-focused, trauma-informed, and committed to social justice.

Key Responsibilities:

  • Facilitate Multidisciplinary Meetings: Organize and lead meetings with various stakeholders, including families and service providers, to ensure effective service coordination.
  • Engage with Community Partners: Collaborate with families and formal partners to create a culturally sensitive and strengths-based service planning process.
  • Conduct Initial Assessments: Perform comprehensive evaluations with youth and referring parties to identify needs and connect them to appropriate services.
  • Provide Immediate Support: Deliver short-term assistance, including case management and psycho-education, in a safe and affirming setting.
  • Orientation and Training: Support the training of staff and community partners on program policies and trauma-informed practices.
  • Collaborate on Interventions: Work with various agencies to enhance screening and support services for youth affected by exploitation.
  • Participate in Supervision: Engage actively in individual and group supervision and training sessions.
  • Travel Requirements: Be prepared to work from various locations and travel as needed within the designated service area.
  • Involve Youth in Care Planning: Assist youth in articulating their treatment goals and navigating available resources.
  • Support Program Initiatives: Contribute to special projects and tasks that align with the goals of the C-Change program.
  • Other responsibilities as assigned.

Qualifications:

  • Bachelor's degree in a relevant field such as psychology or social services.
  • A minimum of three years of experience in case management and rehabilitation services for youth.
  • Experience working with high-risk populations, particularly those who have experienced exploitation or severe mental health challenges.
  • Strong understanding of the needs of foster youth and relevant administrative guidelines.
  • Ability to effectively engage clients and collaborate within diverse teams.
  • Familiarity with community resources and trauma-informed care practices.
  • Proficient in using technology and understanding Diversity, Equity, and Inclusion principles.
  • Valid California driver's license with a clean driving record.
